Linda Ready, RN (EC)
Linda practices as our FHT nurse practitioner in Bobcaygeon. She has a Bachelor of Nursing Science from Queen’s University and a Masters of Nursing and Nurse Practitioner certificate from Athabasca University. Linda's nursing experience across Canada includes adult and pediatric critical care, air transport, critical care education, post anesthesia care and cardiovascular research.
Linda has acted as our Clinical Practice Coordinator and a family health registered nurse.
Rona Formosa, Medical Receptionist
Rona joins our Family Health Team having provided temporary / relief medical receptionist coverage for us these past few years. Having worked in a medical office environment for over 20 years, both Rona’s experience and her excellent demeanour are a wonderful addition to our Family Health Team. Rona currently provides administrative support to our Nurse Practitioner at our Bobcaygeon office.

Patti Thompson, RN (EC)
Patti provides care to patients at the Fenelon Medical Centre. Patti has extensive clinical experience in general medicine and has worked in home care. Before coming to practice in Fenelon, Patti worked in community health care and at Sunnybrook. Patti has training in respiratory care and is a certified foot care specialist. Originally from Toronto, Patti is planning soon to call Coboconk home.
Kylie Pankhurst, B.Sc.N., RN
Kylie practices family nursing at the Fenelon Medical Centre, having worked previously in the surgical unit at the hospital in Peterborough. Kylie has also worked at Pinecrest Nursing Home in Bobcaygeon and has clinical experience with children and new moms. Kylie holds a degree in community health from Brock and a nursing science from Trent University. She grew up in Fenelon and now lives in Bobcaygeon.
Almost everyone in Fenelon knows Carolyn. An experienced medical receptionist, Carolyn provides administrative support and patient care with our team’s nurse practitioner at the Fenelon Medical Centre. Carolyn has also worked as a team leader and laboratory manager and been a charge nurse at Pinecrest Nursing Home in Bobcaygeon.

Samantha Henry, RN (EC), BScN, MN:ANP
Samantha practices as one of our team's two nurse practitioners in Lindsay. Sam has worked as a nurse practitioner for several years and brings a broad range of professional experience to patients and our team. She has worked with seniors, children and in public health and has been a clinical care manager. Samantha holds a nursing science degree from the University of Ottawa and a master's in advanced nursing from Athabasca University.
Laura Devine, RN, BScN, IBCLC
Laura holds a Bachelor of Science degree in Nursing from Ryerson University, is a Certified Lactation Consultant and former Certified Community Health Nurse. Laura has a strong nursing background in Pediatrics and Maternity with an emphasis on healthy child development. She has taught clinical at the college level in the Registered Nurse program.
Laura comes to us with office administration experience with a chiropractor and most recently with the Kawartha North Family Health Team. Laura also has experience in retail. She has a diploma in medical administration and a certificate in hospital ward administration, which included medical terminology and transcription. Laura and her family live in Bobcaygeon.

Della Hare, RN (EC) CDE
Della joins our Family Health Team having held various titles during her nursing career at the Ross Memorial Hospital. It was during her time at RMH that Della completed during her Masters in Nursing, leading her to further her career as a Nurse Practitioner in Primary Health Care.
In addition to being a licensed Nurse Practitioner, Della is also a Certified Diabetes Educator and brings with her a wealth of experience in general medicine, surgery, and critical care emergency nursing.
Yvonne comes to us with considerable experience as a medical receptionist in a family practice in Lindsay. Most recently, Yvonne has been at the Kawartha North Family Health in Fenelon Falls where she worked directly with a nurse practitioner. Yvonne is a certified medical secretary with a passion for helping patients at all stages of the health system. Yvonne and her family live in Lindsay. Yvonne is working in practice with our Nurse Practitioner.

Maureen comes to us having completed her Bachelor of Science in Nursing at the University of Toronto as well as her Bachelor of Science in Human Kinetics at the University of Guelph. She has recently completed her Masters of Nursing through the University of Toronto. Maureen brings with her more than 11 years of experience having previously worked in the Emergency Department at the Peterborough Regional Health Centre.
Carolyn Fallis, Medical Receptionist
Carolyn comes to our Family Health Team with 20+ years of administrative experience working in a health setting. Carolyn is a local resident of Lindsay and brings with her a wealth of knowledge when it comes to local resources as well. We certainly anticipate that Carolyn’s years of expertise in providing exceptional administrative support will be a definite asset to our Nurse Practitioner and patients at our Little Britain location.

Leanne Crump, RN(EC)
Leanne's family health team practice is located at our location at the Omemee Medical Centre. A mid-career professional with experience as an independent nurse practitioner in Lindsay, Peterborough, and Fenelon Falls, Leanne also has specialized knowledge in the areas of pain management, palliative care, child and maternal health and community care. Leanne was instrumental in developing the nurse practitioner program in the primary health care clinic at the Ross Memorial Hospital. She has been an active member of the Primary Care Working Group of the Local Health Integration Network and has coordinated education on nurse practitioner practice.
Summer works at our Team's location at the Omemee Medical Centre, where she arranges appointments and patient care with our Team's nurse practitioner there. Summer is a long-time resident of the Omemee area, with medical office experience and a diploma in medical terminology from Sir Sandford Fleming College.

Heather is the administrator and medical receptionist for our team's family health programs. A native of Fenelon Falls, Heather comes to us with several years of experience as former medical receptionist at the Fenelon Medical Centre. Heather was a registered nurse by profession for many years. She and her family live in Fenelon Falls.

Debbie McLeod, B.Sc.N, ENCc, RN
Debbie is the coordinator of our team's many family health programs. Recently completing her nursing degree where she focused on community health programs, Debbie has many years of experience at the Ross Memorial Hospital including as team leader in the emergency department and administrative coordinator of the nursing division. Debbie’s special interest is health promotion. She and her family live in Lindsay.
Roxanne Davies, RN, IIWCC
Roxanne assists in delivering our team’s family health programs and leads the smoking cessation program. Now from the Janetville area, Roxanne has been with our family health team since its inception in January 2011. She comes to our team from Kapuskasing where she worked as an operating room and obstetrical/surgical nurse.
Beth Syvret, R.Ph., Pharm.D.,
Beth comes to our team from Peterborough where she most recently held a position of pharmacy manager. She has worked as a pharmacist at the Ross Memorial Hospital and as a clinical pharmacist with the GAIN Geriatric Clinic. Beth also brings a decade of experience working in retail/community pharmacy to her work with our local physicians and patients.
Tanya Nye, M.Sc. O.T., O.T. Reg. (ON)
As our team's occupational therapist, Tanya sees patients to help them maximize their health within their environments, include through assessing home and work safety, increasing mobility and promoting injury prevention. Tanya also works with seniors to remain in their homes for as long as they are able. Tanya has focused her career working in the community and has experience working in the auto sector and home care, assisting people of all ages return to their daily activities. Tanya is from Dunsford and comes to our team with a Master's degree in occupational therapy from Queen's University and an undergraduate degree in human kinetics.
Cindy Billiaert, RRT, CRE
Cindy sees patients for education on respiratory disease processes, drug and inhaler therapy, self-management and environmental controls. A graduate of Algonquin College, Cindy comes to our team with experience at several Toronto hospitals, including The Hospital for Sick Children. Cindy is active in asthma/COPD education and pulmonary function testing. She lives near Manilla.
Wendy Lenters, RN, Foot Care Specialist
Wendy comes to our team with a background in medical and geriatric nursing. Wendy holds a certificate in advanced foot care nursing. Wendy is passionate about chronic disease management and advocates that foot care is necessary to prevent illness and maintain overall good health.
Atheana Brown, B.Sc., RD
As our team's Registered Dietitian, Atheana helps patients assess their nutritional status and provides professional advice in order to maximize their health and best manage existing health problems. Atheana comes to our team with experience in various health care settings including: public health, a family health team, outpatient counseling, and hospitals. She holds a science degree from the University of Western Ontario and did her dietetic internship with the Northern Ontario School of Medicine. Atheana is from just outside Lindsay and also leads our team's Healthy Lifestyle Coaching program.
Erin Jenkins, B.Sc., RD
Erin comes to us from the Campbellford Memorial Hospital where she has been working as their Clinical Dietitian on both an Inpatient/Outpatient basis. Erin attended the University of Guelph where she obtained her Bachelor of Applied Science, Honors in Applied Human Nutrition. Erin has taken the reigns as our FHT’s Registered Dietitian, while Atheana Brown is off on a leave. Erin also co-facilitates our FHT’s Craving Change program.
Mental Health Team
Dr. Greg Sweitzer, Psy.D., C.Psych.
As our team’s clinical psychologist, Dr. Sweitzer conducts psychological assessments and provides psychological treatment for adults for depression, anxiety, phobias, trauma, addictions, exploitation and abuse, relationship issues and bereavement. Dr. Sweitzer is also involved in our team's stress education and pain management programs. He comes to our team with 30 years of clinical experience, including 20 years as a social worker. Originally from the United States, Dr. Sweitzer now calls Lindsay home.
Lisa Glover Carmichael, M.S.W., RSW
Having worked locally in the field of education for many years, Lisa comes to our team having conducted mental health assessment, treatment and service provision to adults and youth in community and hospital outpatient mental health settings in Haliburton, Minden, Cobourg and Lindsay. She has worked with children, youth and families, and with women of domestic violence. Lisa lives with her family in the Lindsay area.
Lorraine Gregson, M. Ed., RSW
Lorraine has worked in a variety of clinical counselling settings, including with the Peterborough Family Health Team and Sir Sandford Fleming College. A resident of Bobcaygeon, Lorraine brings more than 20 years of experience in psychotherapy with adolescents, adults and groups to our team. A trained mediator, Lorraine also provides information on community resources and social supports to patients.
Lisa Ann Janveaux, M.S.W., R.S.W.
A registered Social Worker, Lisa Ann come to us with over 25 years of experience working with children, youth and adults in a variety of medical settings. She has run a private practice as well as working in Psychiatric Hospitals and Family Health Team settings. She has extensive experience working in rehabilitation, palliative care, adult mental health and with the dually diagnosed. Lisa Ann brings a friendly smile and positive attitude along with her years of experience, imperative to successful Case Management. She is a willing advocate for her clients and her past experiences are an asset while assisting them to navigate the system.

Elizabeth practices family nursing in collaboration with doctors in Lindsay. A registered nurse since 2004, Elizabeth brings to our team a variety of experience including in the areas of community nursing, long-term care, and geriatric medicine/rehabilitation therapy both in our area and in British Columbia. Elizabeth also has a depth of knowledge in wound care and IV therapy.

Ashleigh completed her nursing degree through York University and comes to our team with significant previous experience in cardiac intensive care. She is also knowledgeable in the areas of diabetes care and a variety of heart conditions. Ashleigh is especially interested in the quality of health care at the community level.

Jean Walsh, Operations Manager
Known as “the keeper of knowledge” Jean is responsible for ensuring the many aspects of our team run smoothly, including day-to-day operations, administrative issues and assisting staff at our team’s eight locations in the community. Living in Lindsay, Jean has been an executive assistant both for the Chief of Police of the City of Kawartha Lakes and in the senior ranks of the private sector. Jean has also worked as Logistics Coordinator, Document Controller and Senior Operations Coordinator with national and international companies.
Kim Smith, CHRP HR Manager
A certified human resources professional, Kim joins our team following many years of human resources experience with the Canadian Mental Health Association Kawartha Lakes Branch. As Human Resources Manager, Kim was involved in all aspects of human resources management including staff supervision, coordinating workplace health and safety and providing leadership support for workplace wellness. She was also a leader in the organization's accreditation process. Kim has additional experience in financial administration. Kim and her family live in Fenelon Falls.
Krissy Hickson, Finance Manager
Krissy holds an honours diploma in accounting and payroll administration from Trillium College and has a wealth of financial management experience primarily in the private sector and with the Canadian Mental Health Association, Kawartha Lakes Branch. When not doing FHT finances, Krissy is the Executive Director of the Fenelon & Area Independent Living Association in Fenelon Falls. In addition to her expertise, Krissy brings with her an energetic, friendly personality that will ensure she is a great match with our team.
Allyssa Adams, Executive Assistant
Allyssa comes to us with experience as an Administrative Assistant at the Ontario Early Years Centre. She also has experience developing resources in the charitable sector in our area through the United Way and various other not for profit agencies. She brings a wealth of knowledge of information technology, having worked for one of Canada’s largest telecommunications companies for over seven years. Allyssa holds a Social Service Worker diploma with Aboriginal Emphasis from Fleming College in Lindsay and hails from Little Britain.
Mike Perry, Executive Director
A lawyer and social worker by profession, Mike has a decade of experience in program management with the federal government in Ottawa where he also worked in communications with the YMCA-YWCA and was involved in community health care advocacy and policy development. A native of Fenelon Falls, Mike has been involved with several health organizations in our area over the years. Mike is proud the family health team was awarded Employer of the Year in 2013 and again in 2016. Mike was recognized as "Citizen of the Year" for Kawartha Lakes in 2017. Mr. Perry is an alumni of the emerging leaders executive education program at Harvard University.
