To become a patient of a family doctor or nurse practitioner of the City of Kawartha Lakes Family Health Team you must first register with Health Care Connect.

Health Care Connect is a Government of Ontario program that identifies and places unattached patients with primary care providers, including family doctors and nurse practitioners.

Health Care Connect will refer people without a family doctor to our Team’s physicians and nurse practitioners as they accept new patients.

After an individual has registered with Health Care Connect a registered nurse called a Care Connector will be assigned to you and assist you in finding a health care provider in our area.
