Family Health Teams
Funded by the Ministry of Health and Long-Term Care, Family Health Teams are a new way of providing health care to you and your family in Ontario.
Family Health Teams are health care organizations that bring together family doctors, nurse practitioners, registered nurses, social workers and other health care providers to coordinate the best possible care for patients in their communities.
With everyone working together, Family Health Teams combine the expertise and services of their doctors and health care providers to meet your needs and provide the best possible and most timely care . Family Health Teams focus on keeping you and your family healthy, as well as treating you when you are sick.
Family Health Teams help to reduce the number of patients who do not have a family doctor.
There are currently 200 Family Health Teams in operation or development in Ontario, improving access to health care for more than an estimated 2.7 million Ontarians.
